Rejuvenation “Down There”

From incontinence to looseness—Dr. Annebelle Aherrera assures that there is a way to make women feel better

WHEN a 65-year-old patient consulted aesthetic gynecologist Dr. Annebelle Aherrera, she was a picture of ageless beauty, with a firm jaw line and smooth skin. However, upon pelvic examination, her vulva, female external genitalia, was sagging and wrinkled. After having it restored to its youthful form, the patient returned to Aherrera’s clinic for radiofrequency therapy for lubrication, to address vaginal dryness.

“A woman has two faces— one up and one down below,” cites Dr. Aherrera. “Many women will go to great lengths just to make their face look younger, without realizing that their ‘face down there’ is lagging behind.”

In 2007, Dr. Aherrera pioneered in hospital-based vaginal aesthetic rejuvenation, a term coined by Dr. David Matlock, the founder of the Laser Vaginal Institute of America. To supplement her practice, she later trained under Filipino-American Dr. Red Alinsod in California. The pioneer in non-surgical aesthetic procedures on the exterior female genitalia and the vagina, he invented ThermiVa, a temperature-controlled radiofrequency non-invasive FDA-approved treatment for non-surgical options.

Dr. Aherrera ventured into this non-surgical procedure that helps address three common dysfunctions that women face. “First, due to the process of aging, women experience vaginal dryness, looseness and reduced vaginal secretion. Second, muscles of the vagina become lax after pregnancy, childbirth and simply aging. This results in stress urinary incontinence—involuntary passage of urine upon coughing, laughing, sneezing and straining. It is a functional impairment because women are not able to engage in their normal physical activities and have to be near a bathroom.

“Third, aesthetic imperfections from sagging vulvar labial skin are addressed. Interestingly, at least 50 percent of women have one of these dysfunctions,” she cites.

Dr. Aherrera explains that as in any part of the skin, there are the collagen and elastin fibers responsible for maintain the skin’s strength, firmness and shape. The collagen provides the skeletal framework of the skin, so it becomes firm. Elastin fibers are intertwined with this to provide resistance to the connective tissues. Aging slowly disconnects the collagen and elastin fibers, resulting in sagging.

ThermiVa, as the name implies, uses the science of heat to wake up the collagen. “After the first session, it allows the collagen to contract so that it starts working. Immediate results—as in firmness of the sagging labia—are seen. On the second session, the collagen is manufactured and integrated with the elastin fibers. Dr. Alinsod claims that six weeks after the first session, you will already experience the desired results. The third session maintains the integrity of the collagen fibers,” says Dr. Aherrera.

She adds that the ThermiVa probe is a delicately designed S-shaped hand piece that follows the curvature of the vaginal canal. The warm heat massages the area being treated, allowing increased blood flow to the labia and clitoris along with the vaginal mucosa, resulting in tightening and improved moisture.

Hence, the treatment is a comfortable 20 minutes—10 minutes to gently massage the vulvar area, and 10 minutes in the vagina.

“There’s no downtime, no pain, no blisters. Women can resume normal activities, including sexual intercourse,” she says.

Her patients have since reported better quality of life. Dr. Aherrera’s first ThermiVa patient experienced two long orgasms after the third session, a secondary benefit experienced even when she only came for vaginal dryness.

“This is because ThermiVa stimulates the vulvar structures, the area of the clitoris, and improves blood circulation,” she says.

Another patient reported that her husband asked why the wife’s opening was taut again. She didn’t divulge her secret.

“The friction during intercourse is heightened because of the tightening of the upper and lower vaginal tissues. When the anterior wall of the vagina is strengthened, the G-spot becomes more accessible,” says Dr. Aherrera.

The oldest patient, 82, suffered from incontinence. Although surgery is recommended to restore pelvic support, she refused at her age. Hence, the doctor combined estrogen cream and ThermiVa. The patient is relieved knowing that her condition could still be addressed.

Before I end this post, let me tell you about a little story. When I was in elementary school, when my period had just arrived, I was scared. I found a book that was sort of a Teenager Girl’s guide to her changing body, what to expect and how to manage (I forgot the title). Of course, my parents looked at me with a weird look but still bought it for me because they were supportive of buying books so I can learn to love reading.

I think it did me a lot of good, you know. I’m not ashamed of bringing up things that would usually earn a hush due to conservatism. I prefer being safe than sorry, much like that trip to check if I had breast cancer. That book was truly a huge help, it explained what would happen to a teenage girl’s body, in the form of comics. There were even menstruation trackers, activity pages, diet suggestions to control acne and more!

I truly hope that I can raise my daughter in an environment where she would feel no need for shame and be open in discussing her intimate concerns with me.

Feel Like a Virgin: The Science of Intimate Makeover

During the event entitled Intimate Affairs held at the Sionil Jose Function Room, The Peninsula Manila last November 29, 2017, Dra. As I was writing this article for my blog, I could not stop myself from laughing and wondering if someone would read this since it tackles a private part and to others, a sensitive topic I gave birth at 23 yrs. old and got five kids when I turned 30. As they say, I was able to utilize my womb to the fullest. After my fifth child and maybe because of my age; I felt that there is something missing. A friend of mine suggested a vaginal repair. Last Tuesday, health writers — all of them female ‘and curious — were invited to an “intimate media briefing” by the Aesthetic Surgery Center of the Makati Medical Center. Held behind closed doors at the MMC boardroom, the lunchaffair promised a long afternoon of intimacy but, of course, not in the lovey-dovey Valentine’s Day sort of way. Spotting a few familiar faces in the room, I settle comfortably in my chair as I’m introduced to Annebelle Aherrera, MD, FPOGS, obstetrician-gynecologist. Over a hearty, healthy fare of pumpkin soup, prawns thermidore, chicken cordon bleu, fettucine with putanesca sauce, and fresh fruits prepared by MMC’s dietary department,

Dr. Aherrera shares out loud an intimate secret (which is really no secret anymore as a lot of women in America are doing it): Laser Vaginal Rejuvenation® with Designer Laser Vaginoplasty® to complement it. When the patient has a “relaxed” vagina, the muscles are no longer firm and have poor tone, strength, and control. When that happens, the vagina is not in its peak condition. Women who suddenly burst into laughter, sneeze, or cough may experience involuntary urine loss or spillover. More often than not, women are embarrassed when these things happen. LVR effectively decreases the internal and external diameters of the vagina, building up the perineal body (the area immediately outside the vagina and above the anus). With this procedure, women feel attractive vaginally, young again, arid they regain their sexuality. Why do women experience vaginal relaxation? As a woman’s body experiences childbirth and the natural aging process, the muscles, fascia, and ligaments separate due to the trauma of childbirth and may be weakened by the pressure. This causes the pelvic organs, composed of the vagina, uterus, bladder, and rectum to drop from their natural positions. But vaginal relaxation is not exclusive to women who have given birth. “EVERYTHING IS VANITY,” King Solomon said. But for people who undergo some form of reshaping or reconstruction of any part of their faces and bodies, the reason may not be quite as simple as that though the publicity over the various procedures high-profile personalities proudly announce may give that impression.

Dr. Manuel O. Fernandez, head of the Makati Medical Center’s Aesthetic Surgery Center (ASC), said for some people, what is referred to as cosmetic surgery may be important to their “psychological health.” Sometimes reshaping a nose may be all it takes to boost a person’s self-esteem and self-confidence. “Any improvement can help improve a person’s confidence,” Fernandez said. Knowing a procedure could seriously change a person’s life, Fernandez said the professionals at the ASC did not simply do what patients asked. People who insist they want a nose like Tom Cruise’s or Angelina Jolie’s will have to go elsewhere because, unless the ASC professionals feel that their faces will match such a proboscis, they cannot have it. Fernandez stressed the ASC would not simply give a patient what he/she wanted if it knew it would not be right or appropriate. There is, for instance, a limit to the extent breasts can be enlarged without causing serious medical problems like severe back pains. Believe it or not, large boobs are a heavy load to be lugging around everywhere. A French woman who used to hold die record for having the largest breasts reportedly could not sleep lying on her back because her boobs were putting a heavy pressure on her chest making it difficult for her to breathe. It is also for reasons other than vanity that ASC hopes women will consider the newest procedures it is offering, the Laser Vaginal Rejuvenation® (LVR®) and Laser Vaginoplasty.

The LVR® decreases the internal and external diameters of the vagina, almost returning it to its original state thus minimizing the problem. Aherrera said vaginal relaxation occurred because of childbirth and aging. But even women who are still young and have not given birth may have the problem. “Women who have inherited weakness of the supporting tissues, those who have experienced unusual strain from chronic coughing or have suffered obesity, may [suffer from] relaxed vaginal tissues,” the expert said. For normal women Aherrera said LVR® was suitable for women with normal sexual functioning. But, it is not “for patients with sexual dysfunction such as those who experience discomfort during sex or those who do not achieve orgasm,” she stressed.

Laser vaginoplasty is very much like rhinoplasty or the nose job except that it is performed on the opposite end of the body. Aherrera describes it as, “a group of aesthetic enhancement procedures of the vulvar structures.” She said, “Many women are embarrassed by the appearance of their external genitalia which are more often than not the result of childbirth, trauma, aging or hereditary characteristics.”

Designer Laser Vaginoplasty®, she added, repaired, enhanced and beautified the genitalia to make it more attractive and youthful looking. Fernandez stressed that while the aesthetic procedures offered by ASC might be available in “stand-alone beauty enhancement facilities, the center had the advantage of being hospital-based. He said some of the reported botched and fatal liposuction procedures in some “beauty clinics,” for instance, could have been averted simply by having the proper equipment for giving the patient oxygen. He added that “ASC only had certified surgeons performing the job.” Being a doctor does not mean you are a specialist,”Fernandez pointed out. One has to be certified by the right bodies before he/she can perform aesthetic surgery. Fernandez said many of the high-profile beauty experts did not do the actual surgery themselves because they did not have the proper credentials.
